Nuke pam_ssh(8). It was moved to DPorts

Having it as security/pam_ssh solves several issues when using multiple LibreSSL/OpenSSL versions. Also this module poses a security risk for unsuspecting system admins, because users might create private ssh keys using very weak passphrases.
